Credit Card Authorization Confirmation

We can ship your gift mugs order to almost any address, worldwide.

However, if the destination address is NOT the same as your credit card billing address, we have to obtain your signature, indicating that it's OK to ship to a different address.

For this you can send us a:

• Fax for which you can find the
  form and details here;
   

• Use this Adobe Acrobat .pdf file
  for easy e-mailing;
   

• PDF copy of the same form
  (in the Adobe Acrobat Portable Document File format - also see below)
   

• Word document of the same form and send it to us as an attachment to an e-mail.

Please note that all above choices require you to send us the form with
your signature
on it as otherwise your credit card company might not give us authorization to ship your gift mugs order to an address that is not your regular billing address.
