Shipment with Customer's FedEx Account

In this case the following Handling & Packaging charges apply per mugs ordered:

• 1 to 4 mugs: $5.00;
• 5 to 12 mugs: $10.00;
• 13 to 36 mugs: $15.00.

Please note that FedEx will only accept your account for regular FedEx shipments.
FedEx Ground cannot be shipped on customer's account.

Note: Should the last digit in the last single-digit data entry field be a Zero "0", then please substitute it with the letter "a" or form may not process.
